Please remember that “sports physicals” performed at other sites are limited evaluations and do not constitute a yearly comprehensive health care visit.
If sports physical is needed, please give the office a call at (516) 771-8123. Please be aware if you have already had a physical, some insurance companies will not cover an additional physical exam within the same year. Please give your insurance company a phone call to inquire regarding this. Please retrieve the reference # and the representative's name and provide us with this information , as this is imperative proof of coverage.
We encourage you to bring your sports approval forms to us for completion in accordance with the American Academy of Pediatrics and the American College of Sports Medicine.

It is imperative to give us a call at (516) 771-8123 for the referral before competing the specialist's appointment as most insurance won't backdate, leaving that visit to your responsibility.
Refilling Your Medications
