How do I schedule an appointment for the Flu Clinic?

After submitting the form, please allow 1–2 business days for our Long Island Pediatrics team to review your request and schedule your appointment!
Once your appointment has been confirmed, you will receive a confirmation text message with the exact date and time of your visit!
Are Parents/ Guardians able to receive the Flu Vaccine at the same time as our child[ren]?
Yes! If parents or guardians are interested in participating, please indicate this in the intake form by selecting “Other” and providing the full names of those interested. Be sure to also include the preferred form of payment.
Please note: Payment will be collected upon arrival. The participation fee for parents/guardians is $25 if paying with cash or $30 if paying by credit card.
Can I discuss other medical concerns or request paperwork during my flu vaccine appointment?
Unforutnately, no. Flu vaccine appointments are designated exclusively for administering the vaccine in order to keep the process quick and efficient for all families.
